Survey Blunder Shocks Parents of Deceased Children

Health authorities in the Västra Götaland region have caused outrage with a questionnaire on child care that was also addressed to parents of deceased children. According to daily Göteborgs Posten 100,000 letters were sent out in order to gather statistics on the acceptance of local care facilities. 300 of those letters reached parents of children who had died as early as 2005. An official responsible for the survey said that the regrettable incident had been caused by a computer program miss.
